Reimbursement:

Coverage is based on reasonable and customary charges.

Lenses and Frames:

Up to $400.00 once every 24 months or 12 months for children under age 18. This would include one pair of Prescription Frames and Lenses (including Tinting and Scratch Resistance), or one pair of Prescription Contact Lenses, or one pair of Prescription Sunglasses. In the event that a Member submits a claim for a Retinal Scan which was prescribed by an Ophthalmologist, the overall maximum will be increased to $450.00.

Eye Exams:

Up to $100 every 24 months, for all adult members. Note: The Ontario Health Insurance Plan (OHIP) is the primary payer of 1 major eye exam (for vision and general eye health) every 12 months, plus any minor assessments you need, if you are 19 years old and younger or 65 years old and over.

Optical Coherence Tomography Picture/Exam:

Up to $100 every 24 months, effective February 1, 2021.

Prescription Disposable Contact Lenses:

Up to $100.00 every 6 months within 24-month period.

Prescription Cataract Lenses (including Physician Measurements):

Up to $500.00 per lifetime, effective January 1, 2024.

Prescription of Safety Glasses:

Up to $200.00 every 24 months, effective May 1, 2018 (not applicable to dependents).

Laser Eye Surgery:

50% up to $1,000.00 every 5 years, effective January 1, 2020.
